You have until September 30th to claim your CME certificate. The certificate will be delivered electronically after you have completed the online evaluation and credit claim procedure. Your feedback is an important part of the CME accreditation process and helps improve future educational offerings. The process only takes 5-10 minutes:
- Go to www.surveymonkey.com/s/ICS2013Evaluation
- Complete the anonymous online evaluation
- You will be automatically directed to the credit claim system
- Login with your unique User ID (included in your registration package, on your badge, and in the email sent after the event)
- Indicate the number of hours you attended
- Your CME certificate will be automatically emailed to the address provided
- Retain the certificate for your personal records
